The Cottage Home. She has a template and great tutorial. And who doesn't love using up fabric scraps. This was the first time that I used heat - n - bond; it's my new addiction!

All of the patterned fabrics are by Amy Butler (scraps from previous projects). I bought the plain white shirt from Target for $6. & I used some leftover turquoise embroidery thread from a project a long time ago!
